Direct Primary Care (DPC) is a subscription-based model designed to make primary care more transparent, accessible, and personalized.
Unlike traditional fee-for-service care, where doctors bill insurance for each service and patients may face unexpected costs, DPC uses a predictable monthly membership fee with no hidden insurance-related fees.
Membership typically includes direct access to a primary care physician, longer and unhurried appointments, and more personalized care. Some DPC practices also offer discounted laboratory services. Pricing is usually set to match market rates or based on services provided by each practice and may vary.

New patient appointments are typically 1-3 weeks to schedule. Follow up appointments for established patients are scheduled based on assessment. Same- and next-business day sick visits are also available for active patients.

You can be a patient at North Star Medicine with or without insurance.
If you have insurance:
You can continue to use your insurance for labs, imaging, medications, specialist visits, and other healthcare costs. Visits with Dr. Tran are included in your monthly membership. In most cases, our negotiated direct prices may be lower than using insurance.If you don’t have insurance:
DPC can be a convenient option for patients without insurance. Dr. Tran can address most primary care needs, while transparent membership and direct pricing make healthcare costs easier to plan. We also offer discounted labs and imaging.North Star Medicine is not insurance, but DPC may complement a high-deductible, health-share, or catastrophic coverage plan.
Currently patients with Medicare and Medicaid are not eligible for enrollment due to limitations of federal policy. This is subject to change in the future.

For patients with a membership at North Star Medicine, there are no per-visit fees for telephone, virtual, or in-office medical appointments. Learn more here.
Given time intensive requirements for In-home visit, availability and cost will be discussed per patient basis.
Labs and imaging may include a small surcharge to offset the charges to the practice by each lab, such as credit card surcharges. Lab and imaging prices vary based on the study and the company providing the service however cost in general will be drastically less expensive given we reveal wholesale price to our patients.

Yes. Dr. Tran is comfortable with prescribing controlled substances; however, never on the first visit. He prescribes based on his clinical interpretation. Understand, that Dr. Tran often focuses on reduction of polypharmacy and deprescription where possible. In cases of palliative and hospice care, Dr. Tran does not shy away from providing patient’s the best standard of care and quality of life.
Standard Prescribing practices are required per federal and state policies (e.g. 3-6 month routine evaluations or more based on patient health status).
All patients sign a controlled-substance agreement during initial enrollment.

Absolutely. Dr. Tran will address your concerns whenever possible and refer you to a specialist when needed—or whenever you would prefer specialty care.
Well-trained family physicians can manage the majority of patients’ medical needs while recognizing when specialty care is appropriate. When a referral is needed, Dr. Tran will help prepare you for the visit with targeted questions and discussion to maximize your time with the specialist.
We also make an effort to know the local healthcare resources so we can help you find options that fit your needs.

You are welcome to use your insurance and take lab or imaging orders to a facility of your choice. However, pricing may vary and can be difficult to predict.
North Star Medicine has a direct agreement with lab companies for significantly discounted cash-pay labs. For example, a basic lab panel may cost around $20, depending on what is ordered.
We also work with local imaging centers that offer cash pricing for services such as X-rays, CT scans, and MRIs.

Privileges may change from time to time. North Star Medicine is independent from hospital systems. As of current, Dr Tran does not provide inpatient and emergency medicine services.

Concierge care often has a higher price point, bills insurance in addition to your monthly or annual subscription (sometimes called a retainer), and often offers 24/7 care.
North Star Medicine is a direct primary care practice, striving to make high-quality care available to families and businesses. We do not bill insurance. Dr. Tran is like having a doctor in the family - he does his best to return the rare, urgent after-hours call, but does not promise 24/7 care (he enjoys time with his family!).
In other words, if it feels like concierge, then I’d say we are doing our job the way we intended it.

Yes! Starting January 1, 2026, eligible patients can use HSA funds to pay for qualifying direct primary care (DPC) membership fees.
This change comes from Section 71308 of the One Big Beautiful Bill Act (Public Law 119-21), signed into law on July 4, 2025. The federal law allows eligible individuals enrolled in qualifying DPC arrangements to use HSA funds tax-free for their DPC membership fees while remaining eligible to contribute to an HSA.
For 2026, qualifying DPC fees are generally limited to $150 per month for an individual or $300 per month for a membership covering more than one person.
This federal provision applies to eligible patients in Ohio and nationwide. HSA eligibility and tax treatment depend on individual circumstances, so patients should consult their HSA provider or tax professional with specific questions.
